Written requests for public records subject to the Freedom of Information Act 5 ILCS 140/ may be made via personal delivery, mail, email, or fax and be directed to:

There is a .15 per page reproduction fee for pages over 50. Oversized, color, or electronic reproduction devices may incur additional fees.
Records stored by electronic means will be provided in the same manner via email if feasible. If records provided electronically are not comprehensible to persons lacking knowledge of computer language or printout format, the requester may contact the FOIA Officer who will, to the extent reasonable and feasible, attempt to assist the requester. Assistance may include providing the requester a key or brief summary of the format of the record to assist the requester in understanding various electronic data codes, or where possible, converting the electronic data to another format that may be easier to understand and/or manipulate by the requester.
